Following a transformative first year of delivering acute care beyond hospital walls, the Hospital at Home program is entering a new chapter, one that deepens its commitment to equity, innovation, and patient-centered care.
Hospital at Home partners with CTCT to support Downtown Eastside residents
Hospital at Home is proud to announce a formal partnership with the Community Transition Care Team (CTCT), a Vancouver Coastal Health initiative supported by the Portland Housing Society. This collaboration marks a significant step forward in expanding access to hospital-level care for residents of the Downtown Eastside (DTES) who are unhoused or precariously housed and unable to safely receive Hospital at Home services in their own homes.
Bringing acute care to supportive housing
The new partnership integrates Hospital at Home’s acute care services into CTCT’s supportive housing environment, enabling vulnerable individuals to complete complex medical treatments — such as extended IV antibiotic therapy — while continuing to receive essential housing, mental health, and substance use supports.
“Hospital at Home is about rethinking how we deliver care, not just the location, but the experience,” says Barbara Drake, Patient Care Manager for Hospital at Home. “This partnership helps us bring that vision to life for people who often face barriers to accessing care, making sure they get the same high-quality support in a way that is delivered around their needs.”

How the collaborative model works
The initiative brings together a multidisciplinary team of Hospital at Home physicians, nurses, and allied health professionals with CTCT nurses, the Addiction Medicine Consult Team (AMCT), and Portland Housing Society staff. Together, they provide coordinated, trauma-informed, and culturally safe care tailored to each person.
Key goals of the partnership include:
- Expanding access to Hospital at Home services for DTES residents.
- Improving occupancy and utilization of CTCT beds.
- Reducing length of stay through streamlined acute care and discharge planning.
- Lowering acute care readmissions by supporting treatment completion at CTCT.
Nursing careers in Hospital at Home
As the program grows, so does the need for skilled and compassionate nursing professionals. Hospital at Home is currently hiring two Registered Nurses (RN) to join this innovative care model.
These roles offer a unique opportunity to work at the intersection of acute care and community health, supporting patients with complex medical and social needs in a non-traditional setting. Nurses will collaborate closely with interdisciplinary teams, providing daily skilled care and helping patients navigate their recovery journey in a supportive environment.

Future plans for expanding care
As the partnership evolves, the Hospital at Home and CTCT teams will continue to refine referral workflows, align communication protocols, and evaluate outcomes. The goal is to expand this model to serve even more individuals who face barriers to traditional hospital care.
